- Doctors: Are my current doctors, specialists, and hospitals covered or in network?

- Pharmacies: Is my preferred pharmacy in network? Is mail-order available?
- Costs: What are the premium, deductible, copays, coinsurance, and yearly out-of-pocket maximum?
- Coverage needs: Do I need coverage when traveling? Do I expect upcoming procedures or ongoing care?
- Extras: Are dental, vision, or hearing services covered, and with what limits?
- Rules: Do I need referrals? What happens if I see an out-of-network provider?
